Endo Advice
One of the most common malpractice insurance claims is for unsatisfactory endodontic treatment. 25% of claims settled relate to treatment of endodontics problems. If in doubt, you risk nothing by asking for specialist advice. Your patients will appreciate the concern and effort on their behalf. You will have greater peace of mind.
Careful evaluation is advised in the following situations:
- Access to the tooth. Some patients have limited opening, which can make access to posterior teeth very difficult.
- Sclerosed canals. These can be difficult to locate and are usually found in the older patient, the heavily restored tooth or one that has suffered trauma.
- Curved canals. These can be quite difficult to deal with and require particular techniques and instrumentation. Without them, ledging, blocking, stripping or perforation of the canal is easily done.

Re-treatment cases. Previous endodontic treatment that has failed may be best dealt with by the specialist. This includes dealing with blockages and perforations, the removal of posts as well as existing root filling materials - including pastes, gutta percha and silver points as well as fractured instruments

- Surgical endodontics. Many cases that used to be treated by endodontic surgery can now be dealt with by disassembly and orthograde re-treatment. The results are more predictable and successful than ever before. Although the need for endodontic surgery has not been eliminated altogether, modern techniques allow it to be drastically reduced.
- Elective endodontics. There are situations where the pulp may have to be sacrificed for restorative reasons, or because of questionable pulp viability. Elective re-treatment may be indicated even though the tooth is symptom less, as crown preparation or re-preparation can disturb the existing coronal seal and cause re-infection of the tooth.
